Rella Aesthetics and Rella Weight Loss are operated by Zachary Wagner, DO.

These Terms of Service (“Terms”) govern your use of rellaweightloss.com, the educational weight-loss assessment, and the online consultation-scheduling experience made available by Rella Aesthetics and Rella Weight Loss (“Rella,” “we,” “us,” or “our”).

By using the website, you agree to these Terms. If you do not agree, do not use the website.

Educational information only

Website content and assessment results are for general educational and administrative purposes. They are not medical advice, diagnosis, treatment, prescribing, medical clearance, or a determination that a medication or program is appropriate for you.

Completing the assessment or scheduling a consultation does not by itself create a physician-patient relationship. A relationship is established only through Rella’s applicable clinical process and professional requirements.

Treatment and medication decisions require an evaluation by a licensed clinician. Not every person is a candidate for medication or treatment. Individual results vary.

Emergencies

The website, assessment, text messaging, and online scheduling are not for emergencies. Call 911 or seek immediate emergency care for an emergency. Do not use this website to report urgent symptoms or serious medication side effects.

Telehealth

Some services may be offered by telephone or another form of telehealth to people located in California. Before Rella delivers health care through telehealth, the provider will explain the use of telehealth, obtain required verbal or written consent, and document that consent.

The website assessment and scheduling steps are not a substitute for that clinical telehealth-consent process. You may ask about an in-person option. Availability and appropriateness depend on the service and clinical judgment.

Assessment and contact information

You agree to provide accurate information and not impersonate another person. Do not submit detailed medical history, diagnoses, laboratory results, insurance information, or other sensitive clinical information through the marketing assessment.

The assessment uses general preferences to organize follow-up. Investment readiness and timing affect only non-clinical lead routing; they do not determine medical eligibility or the care a clinician recommends.

Scheduling

Boulevard is Rella’s source of truth for available consultation dates and times, provider, location, appointment status, and appointment lifecycle. Availability may change before a time is reserved.

Cancellation, rescheduling, deposit, payment, and refund rules are those presented during booking or in Rella’s separately provided patient and payment policies.

Please give at least 24 hours’ notice to cancel or reschedule a consultation. A cancellation or no-show fee may apply when disclosed in the booking or patient policy. Payment terms are those presented during booking or separately provided before purchase. Completed consultations are nonrefundable except where required by law or expressly stated otherwise.

Calls, email, and text messages

When you request contact about an assessment or consultation, Rella may use the phone number or email address you provide to respond to that request. Message and data rates may apply. Reply STOP to stop texts or HELP for help.

Optional promotional calls and texts require separate consent. Promotional consent is not a condition of purchase. Message frequency varies. You may revoke promotional consent through a reasonable method, including replying STOP to a text.

Stopping promotional messages may not prevent Rella from sending a non-marketing communication that is permitted or required by law, such as a message specifically requested by you or certain appointment-related notices.

Privacy

Rella’s Website Privacy Policy explains website data practices. Any separate HIPAA Notice of Privacy Practices supplied to patients governs protected health information within its scope.

Acceptable use

You may not:

Intellectual property

Website content, branding, text, graphics, and software are owned by or licensed to Rella and are protected by applicable intellectual-property laws. You may use the website only for personal, noncommercial purposes unless Rella gives written permission.

Third-party services

The website may use or link to third-party services, including Boulevard for scheduling. A third party may apply its own terms and privacy policy. Rella is not responsible for an independent third party’s content or practices.

Disclaimers and limitation of liability

The website is provided on an “as available” basis. Rella does not guarantee that the website will always be uninterrupted, error-free, or suitable for every purpose.

To the maximum extent permitted by law, Rella, Zachary Wagner, DO, and their affiliates will not be liable for indirect, incidental, special, consequential, or punitive damages arising from use of the website. Nothing in these Terms excludes liability that cannot lawfully be excluded.

You agree to indemnify and hold Rella harmless from claims arising from your unlawful misuse of the website or violation of these Terms.

These Terms are governed by California law. Any dispute not otherwise resolved will be heard in a court of competent jurisdiction in California. If any provision is found unenforceable, the remaining provisions remain in effect.

Changes

Rella may update these Terms. The effective date above identifies the current version. Continued use after an effective update constitutes acceptance only to the extent permitted by law.
